Two years ago, HCF was pleased to launch a new community website called KCHealthMatters.org. The goal of the website is to help strengthen and build healthier communities by providing a one-stop shot for data and resources related to the health of specific communities.
By visiting the website, you can find health data, explore promising practices, locate local reports and identify funding opportunities.
Since we launched, we’ve continued to make improvements. In addition to continuously adding and updating the data featured on the site, we’ve added a SocioNeeds Index that measures the socioeconomic needs correlated with poor health outcomes.
The website also now features a Community Health Needs Assessment Guide to help local public health departments, hospitals, providers and community coalitions conduct community health needs assessments and develop an implementation strategy.
This week we are also proud to introduce a series of new infographics related to KCHealthMattters. We’ve created infographics for each of the six counties in the HCF service area. The infographics demonstrate the resources available through the website and highlight indicators unique to each county that show both progress and also areas needing improvement.
If you are new to KCHealthMatters, we also have some brief tutorial videos to help you better understand how to use the website.
We designed these tools to help our grantees and other community partners learn more about the health needs in their community and provide the tools needed to begin working on projects to improve health.
