Foundation Defined Grants

The Foundation Defined Grants Application has two stages and FDG Applicants must complete both stages: Letter of Intent (stage 1) and Full Proposal Narrative (stage 2). Full proposals are received and reviewed by staff and outside reviewers. Grants are ultimately approved and awarded by the HCF Board.

The application deadline for the 2015 Mental Health RFP Health RFP has passed. Awards will be announced August 12, 2015.

2015 Safety Net RFP

  • Letter of Intent-The Safety Net Letter of Intent Deadline has passed

  • *NOTE-NEW POLICY-HCF is accepting grant requests for up to 2-years with this RFP. If you are requesting funding for more than one year, you must include a strong rationale as to why multi-year funding is essential to your project.

    501(C)3 Applicants Include the following additional attachments:

  • IRS Letter of Determination
  • Copy of your organization’s most recent IRS 990 Report (nonprofit tax return).
  • Most recent audit
  • Board approved current fiscal year operating budget
  • Roster of board of directors with diversity information (race and gender)
  • Governmental entities must include the following additional attachments:

  • enabling statute/legislation or official description of purpose
  • authorized budget or current financial statement
  • list of elected/appointed officials who oversee the entity performance
  • IMPORTANT, PLEASE NOTE: City, Hospital, and University Applicants : Cities, hospitals and universities are allowed to submit 2 proposals per each Foundation Defined Grant (FDG) round; however, if you are unsure whether other departments within your organization have already submitted application(s) , please contact Karen Cornish-Guile at 816.242.0706 to inquire before beginning the lengthy application process. Applicant Defined Grants

Applications for Application Defined Grants (ADGs) will be reviewed on a regular basis throughout the year. Organizations interested in applying for ADGs will be asked to complete the following Electronic Grant Application. Applications must be submitted by 12:00 p.m. (noon) on the first Wednesday of each month, with the exception of December 2015, (the Foundation will not be accepting ADG applications for the month of December.)

NOTE - Organizations seeking continuation grants may not apply any sooner than 90 days prior to the end of your current grant.
