The Lafayette County S.T.O.P. program is requesting support from the HCF to fund a Domestic Violence Court project for 3 years. HCF funding will support treatment and staff for the program. There will be up to 30 clients. Domestic Violence cases will be referred by the courts to the program. At this time, members of Probation and Parole and Treatment Facilitators will provide a 26 week proven national recognized program to hold accountable domestic violence offenders. Counseling for the offenders as well as the victims will occur. This will cut down on recidivism and lessen the load of traditional criminal courts.Geographic Area Served:Lafayette - 100%

The STOP Drug Court Program is requesting continuation funding from HCF to support salary costs, training expenses, equipment/supplies, and the cost of an external evaluation of the program. The Access and Adequate Care project would allow the Health Care Coalition of Lafayette County to subsidize the transportation costs of patients who utilize Rodgers-Lafayette FQHC and the local Rural Health Clinics. We plan to advance to phase II of the research study on a county-wide insurance plan. Our agency continues to address the access to care issues and by providing transportation vouchers to patients, we are directly influencing the rate of follow up care. The primary function of this project is to strengthen the organizational capacity of the Health Care Coalition of Lafayette County (HCC). Through expansion of revenue generating programs the HCC has an opportunity to continue to coordinate a health care delivery system for the uninsured and underserved. One goal of the HCC is to provide health care to small businesses in our county that not only addresses the issue of uninsured but drives traffic to local providers. House of Hope, Inc. Child and Family Support Program provides support to children and their families to navigate the emotionally and physically demanding process of coping with the effects of domestic and sexual violence. Services are provided by a Child and Family Support Specialist knowledgeable in the areas of child development and age-appropriate intervention techniques to empower, educate, prepare, and support children and their families as they transition into a life free of violence.

The Migrant Farmworkers Medical Case Management Project (MFP) in Lafayette County is a unique and highly-effective project that provides medical case managers for migrant farmworkers and their families. HCF has funded the project since 2006.
MFP serves a population identified as 'high risk' because of their mobility (moving to 3-5 locations per year), rural isolation, lack of access to healthcare or transportation, low literacy levels and their cultural barriers and limited English proficiency.
During the past four years, Legal Aid provided medical case management for 2,658 medical and dental visits, or an average of 664 visits per year.

In the Medical Case Management Project, Legal Aid of Western Missouri's staff act as medical case managers for Spanish-speaking, migrant farmworkers and their families who come to Lafayette County each year for the apple harvest. In the Medical Case Management Project, staff from Legal Aid of Western Missouri (Legal Aid) act as medical case managers for Spanish-speaking, migrant farmworkers and their families. Each year, hundreds of immigrant agricultural workers come to Lafayette County to harvest apples and perform related farm work. In three years, the project has assisted farmworkers in 2,044 medical visits. Legal Aid case managers have helped migrant families in Lafayette County overcome language, cultural and transportation impediments that block their access to healthcare. The Brighter Futures Consortium is a new initiative of the six public school districts of Lafayette County. Together the districts account for over 5,700 students and over 560 professional educators. The six school districts have a long history of working together to benefit the educational needs of the students of Lafayette County, with superintendents, principals, counselors and directors of special education meeting routinely to discuss educational issues within the county. Outside of the school districts, staff is collaborating with community agencies, such as the Children's Division and the Lafayette County Juvenile Office, on a monthly basis to better serve student needs. The goal of the Brighter Futures Consortium is to develop and implement methods of preventing and reducing mental health issues among youth ages 0-19, during the regular school day. The Consortium will bring mental health services to students in a familiar school setting and eliminate the multiple obstacles and barriers that currently cause many students to not receive the help they need. This initiative will be accomplished through the hiring of one full-time licensed social worker and a part-time coordinator, and by contracting with appropriate licensed professionals who could include licensed clinical social workers, professional counselors, psychologists, registered play therapists and certified substance abuse counselors.Geographic Area Served:Lafayette - 100%
