Through $194,031 of funding from the Health Care Foundation of Greater Kansas City, the ACCESS program will provide clinical mental health and wrap around care services to students within the Belton School District. Currently our community lacks clinical resources and a majority of our students have profound financial need as well as a high percentage of mental health disorders and inpatient placements. This program will eliminate barriers to clinical care such as lack of availability, transportation, parental support, and financial requirements.

Victims of domestic violence often suffer emotional trauma from experiencing abuse. Hope Haven proposes to contract services from Pathways to provide both on-site and off-site treatment for victims and their children. In addition, Pathways would provide staff training and consultation for staff members when necessary. With the addition of Pathways, we will be able to extend therapy to children and clients with multiple issues which have been unable to access services in the past. Hope Haven of Cass County is applying for funding to establish services for survivors of sexual assault in Cass County. A community response team of law enforcement, health care providers and advocates will be created to meet the needs of survivors. Pathways Community Behavioral Healthcare proposes to serve the adolescent populations and their families residing in both Cass and Lafayette Counties. Through two innovative programs, Functional Family Therapy (FFT) and the Adolescent Community Reinforcement Approach (ACRA), Pathways will be able to serve both substance abuse and mental health needs of adolescents and their families. Funding will be utilized to support staff and staff trainings, resources required to provide service provision, and staff travel. Outcomes will be measured by the number of clients served through both approaches and client satisfaction survey's.Geographic Area Served:Cass - 50%, Lafayette - 50%

Pathways Community Behavioral Healthcare, Inc. is requesting $330,002.00 to continue providing comprehensive behavioral healthcare services to Cass and Lafayette County residents suffering from co-occurring disorders; depression; and family, domestic violence and child abuse. The proposed continuation project provides 4.5 full time mental health practitioners; 1.5 FTE psychiatrist, 2.0 FTE clinicians, and 1.0 FTE nurse. Psychiatry efforts will be targeted towards psychiatric evaluations, prescribing psychotropic medications as well as medication management and monitoring for clients. Clinicians are competently trained in providing Parent-Child Interactive Therapy, Cognitive Behavioral Therapy as well as Family Therapies. Geographic Area Served: Cass-50%, Lafayette-50%

Through two innovative programs, Fighting 2 Preserve Families (F2F) and the Adolescent Community Reinforcement Approach (ACRA), Pathways will be able to serve both substance abuse and mental health needs of adolescents and their families. Foundation funding will be utilized to support staff salaries and Pathways will provide both cash and in-kind match for remaining project related activities. Outcomes will be measured by the number of clients served,improvement in client's mental health well being and client satisfaction surveys.

This program will provide Medicaid children ages 5-18 from northern Cass and southern Jackson counties (Belton, Raymore, Peculiar and Grandview communities) with needed dental care. This program will cover preventative care and dental emergencies. Covered procedures include, but are not limited to: fillings, root canals, extractions, x-rays and cleanings. Expansion to the current Family Planning Program offering birth control with charges based on household income. This grant would enable the program to include IUDs as a long term method of birth control at WCMCAA's two Cass County Clinics (Belton and Harrisonville). Gas Vouchers would be provided to low income clients that would otherwise not be able to access the Cass County clinic.

Through the Headstart, Outreach and Health Services Divisions we are proposing to be able to continue healthy behaviors around the well-known risks of physical inactivity, poor nutrition, and tobacco use for the low-income population of Cass County. Our current program has successfully incorporated family enrichment trainings and education programs through Head Start and our Outreach Divisions. We reach families through our Women's Health clinics with screenings and counseling regarding the risks of obesity and tobacco use. We feel we have a program that motivates and develops skills to translate knowledge to action.Geographic Area Served:Cass - 99%, Jackson - 1%

WCMCAA Women's Health Services is dedicated to providing quality health care to all women, regardless of ability to pay. Services provided include routine pap smears, clinical breast exam, sexually transmitted disease testing and treatment, pregnancy test and referrals, contraceptives and Gardasil vaccinations. When needed, clinical staff assist clients to access additional health care services for their additional health care needs. The majority of women accessing WCMCAA Comprehensive Women's Health Services consider the clinic to be their 'only source of health care'. The overall result of the program is that the health care needs of women who are uninsured or underinsured are met.
