FDG Application Process

Application Process

The application process for Foundation Defined Grants has seven steps:

  1. HCF issues a Request for Proposal (RFP) setting forth the grant priorities.
  2. Potential applicants attend a pre-proposal conference hosted by HCF. The pre-proposal conference is an opportunity to ask questions, receive clarification, and network with other applicants.
  3. Interested organizations submit a letter of intent.
  4. Interested organizations submit an application.
  5. HCF staff and outside reviewers review the applications for completeness. See these helpful application tips from outside reviewers.
  6. The HCF Board of Directors approves the grant awards for the funding cycle.
  7. Awardees will be notified of their funding by letter, by visiting the HCF website, or signing up for our press release email list.

  • Stage 1: Submit a Letter of Intent (Deadline is January 11 at 12:00 p.m. [noon])
    • Begin an online Letter of Intent application
    • Download the Letter of Intent narrative template
    • Upload the following to your online application:
      • Completed Letter of Intent narrative template
      • IRS Letter of Determination (for 501(c)(3) orgs) or enabling statute/legislation or official description of purpose (for government agencies)

  • Stage 2: Submit a full proposal (Deadline is February 15 at 12:00 p.m. [noon])
    • Log into your online account to access your full proposal application
    • Use the proposal narrative instructions to create your proposal narrative
    • Download and complete the budget template
    • Upload the following to your online application:
      • 501(c)(3) applicants
        • IRS Letter of Determination
        • Copy of your organization’s most recent IRS 990 Report (nonprofit tax return)
        • Copy of your organization’s most recent financial audit
        • Board-approved current fiscal year operating budget for your organization
        • Roster of your organization’s board of directors with diversity information (race and gender)
      • Governmental entity applicants
        • Copy of enabling statute/legislation or official description of purpose
        • Copy of authorized budget or current financial statement
        • List of elected/appointed officials who oversee the entity’s performance
      • (Optional) Program logic model & outcome measurement framework examples
      • (Optional) FDG Application Checklist
  • Stage 1: Submit a Letter of Intent:

    • Download and complete the budget templateNote: if you are requesting multi-year funding, you must complete a budget for each year.  Please use the appropriate tabs at the bottom of the budget worksheet for Year 1 and Year 2
    • Upload the following to your online application:
      • 501(c)(3) applicants
        • IRS Letter of Determination

        • Copy of your organization’s most recent financial audit
